How Do I Access Home Care Services?
Patients and/or family members may directly contact Ummah Care LLC in order to access services. You can do the option of Medicaid or out-of-pocket to pay for services. A form will be faxed to your physician. A physician certifies the need for services and establishes a “plan of care” for you. When the above conditions are met services can start by state.
